I use an Invacare Aviva RX 40 (roughly 65cm wide x 120cm long x 145cm tall with headrest). As Mark mentioned, there are London Cab-esque taxis called JapanTaxi that you can flag down in most large cities.
However, even though they were supposedly designed to be accessible, it is pretty obvious they were made with a small manual wheelchair…

Hello, I suggest you get in touch with @ohayotravel and he can maybe help you with the rental, but also introduce you to Abilities – I believe you can visit their showroom in Tokyo.
The batteries in my wheelchair just died and I need two new ones.
Unfortunately I think they are the only authorized repair shop for Permobil… Instead of changing your flight and being stuck in your hotel room, maybe it is best to put it in manual mode and push it to a wheelchair taxi? Then you can have your local shop repair it.
