Japan Rail Pass is very much worth it if you are traveling that far as you can also use it on local JR lines etc.
Using a wheelchair, you will still need to make reservations for the Shinkansen to get the accessible seat with or without the JR Pass. When you have decided on the train you want to ride on, go to a “Midori no Madoguchi”…

I just popped across the street to the drug store and there were both the S-sized senior diapers and the big sized children’s diapers in ample supply. Both are less than 1000 yen for the package.

Diapers are sold at stores here, even adult diapers for seniors. (Though, people with disabilities and seniors can get them for free from the city if they are a local resident.)
