Though it isn’t likely a common travel destination for visitors to Japan, Huis Ten Boschin Nagasaki is fairly wheelchair accessible.
Most stores have ramps and there are accessible toilets. Even the small canal ferry is accessible.
The cobblestone is a bit uncomfortable in a wheelchair though.
The biggest challenge is getting there though as you pretty much must use a taxi. And that can be expensive.
