I have never been and even finding Japanese information on accessibility is proving a bit difficult. The most I have is a place mentioning there is an accessible toilet and ramp, and that one of the private rental baths in “Yasuragi” was renovated to be accessible.
Now, as far as I understand it is more of an amusement-based family facility, not a real onsen (though it does have onsen baths as well). Is that what you are looking for? You can use swimwear at pools, but for baths you would usually go in without.
Or are you more interested in a more traditional onsen?
If you are more interested in a Japanese onsen, something like Hakone Pax Yoshino (https://www.accessible-japan.com/hotels/japan/kanagawa-1/hakone/mid-range/hakone-pax-yoshino/). It has an accessible private bath.
