Unfortunately, I don’t know of any organizations in the nearing countries, but I’ve been able to find a group of people that are helping disabled, elderly, LGTBIQA people, families and more evacuate from Ukraine.

Nevertheless, it’s a new organization, so I’d also try to contact more established ones. For instance, I’m sure that the Ukrainian/Polish/Romanian Paralympic Committees may have a lot of information on accessible transportation in the area:
